Transition

The Next Step Toward Independence

For adolescent residents who have shown personal growth and achieved the behavioral goals outlined in their treatment, we provide opportunities for additional social outings, access to technology and allowance shopping within the safety and supervision of our transition program.

 

Navigating this daily structure, along with its processes and procedures means a higher level of intentional focus, planning and initiative. The transition phase emphasizes creating a purposeful and productive schedule that promotes accountability in addition to executive functioning skills for school, employment, volunteering, mentoring – or a combination of all four. Social outings create opportunities for residents to connect with peers and work on social skills. Allowance shopping promotes budgeting and making wise purchases. Volunteering and employment opportunities teach follow-through, accountability and work ethic, while focusing outside their struggle to help others.

 

By learning new independent-living skills, practicing those they’ve already developed and continuing to focus on their personal growth, our adolescent residents can effectively prepare for productive futures post-treatment.

Therapeutic

  • Comprehensive medication management with weekly medication checks

  • Individual therapy two times per week

  • Family education/therapy weekly

  • Specialty groups twice a day led by highly-specialized  clinicians

  • Psycho-educational groups that include, but are not limited to the following:

  • Mood regulation

  • Dialectical behavioral therapy

  • Life skills

  • Executive functioning

  • Health education

  • Coping skills

  • Culinary classes

  • Expressive art therapy

  • Values and morals

  • Healthy relationships

  • Ethics and world view

  • Goal setting

  • Integral recovery group

  • Formal clinical staffing every other week, led by our full-time Child and Adolescent Psychiatrist with written progress reports and updates given to parents and referral sources

  • Supervised personal health and fitness plans emphasizing the importance of exercise, effective sleep patterns, stress reduction, nutrition and relaxation techniques

  • 24-hour awake staff supervision and support

  • Transportation to all appointments

  • Educational, volunteering or employment opportunities

Education

  • Ability to complete high-school diploma either through the student’s previous school or our fully accredited high school, Innercept Academy

  • GED services and/or college dual enrollment with support from the teaching staff

  • Daily educational groups on schedule maintenance, decision-making, finance, health and fitness, leisure, conflict resolution, healthy relationships and recognizing the importance of attending to daily responsibilities.

  • Ability to facilitate academic testing by experienced licensed clinical school psychologists

  • Classroom time is interspersed with physical activity and community responsibility to provide opportunities to apply knowledge in real-life situations.

Life Skills

  • Help navigating the academic and volunteer opportunities available both internally and externally in the community.

  • Shared programming experience in a home setting to encourage relationship skills with roommates, peers and authority figures.

  • Group sessions that teach the principles of critical thinking and application to life situations.

  • Transportation to academic or volunteer opportunities and all other appointments.

  • Increased access to technology.

  • Increased time in the community.

  • Smaller group size to focus on newly-acquired skills.
